**Case Manager**
**Location:** Remote
**Open to candidates based in:** Pittsburgh, PA, Durham & Charlotte, NC, Phoenix, AZ, Orlando, FL, Dallas & Fort Worth, TX.
**About the role:**
Weβre looking for a Case Manager to support patients and healthcare providers throughout their treatment journey. In this role, youβll manage cases, provide updates, and help ensure patients have access to the programs and support they need.
**What youβll do:**
+ Act as the main point of contact for patients, providers, and internal teams
+ Support program enrollment, benefits verification, and pharmacy referrals
+ Conduct onboarding calls and follow up with patients as needed
+ Provide updates on case status and next steps
+ Process patient assistance program (PAP) requests
+ Help resolve issues and escalate complex cases when needed
+ Work with cross-functional teams to support patient needs
