**Job Summary:**

Assists with the coordination of patient centered and occupational based goals to ensure diagnostic and therapeutic services and rehabilitation are provided to patients.

+ Accurately follows established treatment plan to set measurable and achievable goals to reach maximum functional independence for Activities of Daily Living (ADL's) and Instrumental Activities of Daily Living (IADL's).
+ Accurately records and documents care and changes patient condition.
