Our Reablement team supports individuals following a life changing event or a hospital stay, helping them regain independence and confidence.
What you’ll do
* Deliver a high-quality, efficient Occupational Therapy service in clients’ homes and community settings.
* Provide specialist OT advice, assessment, and intervention to maximise functional ability and quality of life.
* Support reablement by enabling early, safe discharge from hospital and preventing unnecessary care placements.
* Work closely with Social Workers, health professionals, carers, and families to coordinate care.
