As an Occupational Therapist in the Early Support
Discharge Team, you will:
* Play
a key role in helping clients meet their occupational
performance goals following hospital discharge * Work within a
multidisciplinary team to deliver person-centred care in home and
community settings * Consistently collaborate,
sharing expertise and resources to deliver the best possible outcomes for
