**Optum is seeking a dedicated, full-time (40 hrs/wk) Nurse Practitioner to join our HouseCalls team in Cuyahoga & Medina counties Ohio (i.e., greater Cleveland & Medina). Optum is a clinician-led care organization, that is creating a seamless health journey for patients across the care continuum.**

**As a member of the broader Home and Community Care team, you'll provide annual clinical assessments to patients in the comfort of their homes.
