Job Description
- Provide evidenced-based care to a panel of patients in collaboration with a clinical care team.
- Conduct accurate assessment, diagnosis, and treatment management of health problems.
- Perform health counseling and disposition planning for patients.
- Maintain NP license in good standing with no current or prior disciplinary actions.
- Submit required documentation including BLS certification and completed candidate questionnaire.
- Onsite as needed; position involves patient-facing responsibilities.
- Work hours: Monday to Friday 8:30am-7:30pm, Saturday 9am-5:30pm, Sunday 9am-4:30pm.
- This position requires working every other weekend with possible holiday rotations.
