Nurse Practitioner (Full-Time) – Post-Acute / SNF Focus Greater Chicago Area

We are seeking an experienced Nurse Practitioner (NP) to join us as a foundational clinical hire in the Greater Chicago market. This is a unique, early-stage opportunity to help build and shape a consultative clinical model focused on hydration health in skilled nursing facilities, working closely with both Corporate and Clinical Leadership. This role is ideal for a clinically strong, entrepreneurial NP who thrives in a high-autonomy environment and is excited to contribute to something being built from the ground up.
