About the Role

Our Cardiology Nurse Practitioner will provide advanced, patient‑centred care across the continuum of cardiology services within Waitematā District, with a particular focus on heart failure. The clinician will function both autonomously and collaboratively, delivering expert assessment, diagnosis, treatment, and ongoing management for patients with complex and chronic cardiac conditions. The role is integral to improving access, equity, and health outcomes for the Waitematā population by supporting timely, high‑quality care across acute, inpatient, outpatient, and community settings. A key focus is the proactive management of heart failure and other cardiology conditions to reduce avoidable hospital admissions, support early intervention, and enable safe transitions of care between hospital and community services.
