Job Description
Duties
- Manage a caseload of 20-23 patients, conducting initial assessments within 24 hours.
- Coordinate discharge plans and all post-acute care needs.
- Work closely with the medical team, insurance case managers, and Providence UM case managers to ensure safe and efficient patient discharges.
- Facilitate discharge and contingency planning, including crisis intervention.
- Assist patients with navigating the Department of Children and Families (DCF) process.
- Coordinate services for home care and equipment following discharge.
- Document pertinent information to facilitate care coordination.
- Educate patients and caregivers to facilitate discharge and reinforce individualized education.
Skills
- Proficiency in Epic charting system.
