We are looking for a highly organised and people-focused Service Coordinator to join our small, committed Community Living team. Working under the ACC Maximise Independence contract, you will play a key role in supporting kiritaki to live safely, confidently, and independently in their own homes. This is primarily a coordination and service delivery role, where you will ensure services run smoothly day-to-day. You will work closely with the Service Manager and Clinical Coordinator (Registered Nurse) to coordinate supports that align with each client's needs and ACC-approved plans.As a Registered Health Professional, you will also bring valuable clinical insight and have the opportunity to step into broader leadership and service management support when required, contributing to the overall resilience and capability of the team.
