We are seeking to recruit a highly motivated and organised Registered Nurse, Occupational Therapist or Social Worker to the Mental Health Out of Area team (secure).

The team consists of three practitioners who support CWP patients admitted into the secure pathway (High, Medium and Low). Each practitioner holds a caseload of patients and focuses on ensuring a strong connection and local link is maintained while patients are in out of area hospitals. The team aims to build strong links with their MDT and families and support them along with the patient 0s recovery journey in hospital and transition back into the community when ready for discharge.

The team is based at Bowmere hospital. The post holder will be expected to travel across the North West footprint. The Forensic Support Service base is located in Saddlebridge and could be considered as a base if required (please indicate at interview).
