Job Summary:
The Medication Reconciliation Technician plays a critical role in ensuring patient safety by accurately collecting, reviewing, and documenting medication histories. This position supports pharmacists and other healthcare providers in identifying and resolving discrepancies in medication records during transitions of care, such as admission, transfer, and discharge.

Key Responsibilities:
โObtain and document accurate medication histories from patients upon admission.
โGoing into patients rooms with doctors and nurses to obtain history (patient facing role)
โReview and compare current medications with newly prescribed ones to identify discrepancies.
โCollaborate with pharmacists, physicians, nurses, and outpatient pharmacies to clarify medication information.
โAssist in updating allergy information and medication lists in electronic health recor...
