Job Description
Roles and Responsibilities
- Provide counseling service to patients and caregivers
- Provide psychosocial and financial assessment, coordinated case management and interventions to enable the patient/client to function optimally in the home environment and community at large
- Assist patient to understand the diagnosis and implication on ADL functioning
- Walk through with patients and caregivers and provide emotional support and motivation for them
- Manage patient expectations and set realistic and achievable goals
- Arrange for interim care or make referrals for long‑term step‑down facilities when necessary
- Participate in ward rounds and case conferences to represent patients' social, psychological and economic circumstances
- Responsible for volunteer management
- Plan and oversee the volunteer programs, not limited to recruitment, screening, coordination, training and retention of volunteers
