Responsibilities:

. Report directly to the Centre Manager.

. Be involved in the pre-admission work, such as taking on a new referral, liaising with the external agencies and internal team to facilitate an admission to centre based services.

. Assess & evaluate patients'/families' psycho-social and financial status.

. Assist clients to apply for appropriate financial assistance schemes when needed (Medifund, Household Means Testing (Subvention) and other funding schemes).

. Assist in setting up and upkeep of administrative protocols pertaining to patient needs from onboarding to discharging.

. Work closely with team members in the development of the Individual Care Plan.

. Provide accurate information to clients on policies, procedures and service.
