The Senior /Medical Social Worker Assistant supports and assists the Senior / Medical Social Worker (S/MSW) to look into the possible interventions (to receive social and financial assistance, renew subvention of healthcare subsidy and eligible funding schemes) to help reduce the burden of the clients and families from incurred social and healthcare costs.

Work Location: Hougang / Pearl's Hill Care Home

• Regular liaisons with clients and families to follow up on pending assistance – Medifund, Household Means Testing (Subvention) and other funding schemes, etc.
• Be updated with available financial assistance and provide advice to clients and their families on eligible resources to tap on
• Support and adhere to the care plans developed by S/MSW (Centre Manager) and other healthcare colleagues
