**Job Title:** Medical Forms Designer
**Location:** Coppell, TX-on site
**Job Type:** Hourly Consultant
**Hours:** 8 am to 5 pm
**Pay:** $22.80/hr

**Essential Functions:**

+ Serve as the primary resource and advisor for medical record forms document design and revision initiatives.
+ Convert appropriate medical record forms to digital platforms and identify opportunities for digital document development and optimization.
+ Participate as an active member of the client’s Facility Form Committee to review, revise, and develop medical forms as needed.
+ Provide expertise and support for document production requirements across lithographic, web printing, and digital print solutions.
+ Manage customer relationships by advising document owners on cost-effective document production strategies and solutions.
