Job Description
Medicaid Navigator
LaSante Health Center is seeking a Medicaid Navigator to help patients access essential services and support programs. The ideal candidate will assist with screening for social needs, determining eligibility, and connecting patients to resources such as housing, food assistance, transportation, behavioral health services, and Medicaid programsโincluding those offered through the 1115 Waiver. This role requires strong communication skills, attention to detail, and a compassionate approach to patient support.
Responsibilities:
+ Screen patients for Social Determinants of Health (SDOH) needs.
+ Assess eligibility and facilitate connection to Medicaid and community services.
+ Guide patients clearly and compassionately through available programs and benefits.
+ Coordinate with clinical staff and external partners to ensure smooth continuity of care.
+ Document all activities accurately within the EHR while maintaining HIPAA complian...
