Job Description
MDS Coordinator
Manchester Center for Rehabilitation and Healing is looking for talented and motivated MDS Coordinator to join our dynamic team. The position is within a Skilled Nursing facility. Candidate should possess passion for geriatric population, must contain a license in the state of practice, be able to evaluate and treat independently without direct supervision, and posses the desire to perform functional therapeutic activities to assist residents improve in their experience.
MDS Coordinator Essential Job Functions
+ Oversees accurate and thorough completion of the Minimum Data Set (MDS), Care Area Assessments (CAAs) and Care Plans, in accordance with current federal and state regulations and guidelines that govern the process
+ Acts as an in-house Case Manager demonstrating detailed knowledge of residents health status, critical thinking skills to develop an appropriate care pathway and timely communication of needed information to the r...
