Cleveland Clinic Homecare provides skilled nursing, social services and physical, occupational and speech therapy for patients returning home after a hospital stay, outpatient procedure or doctor's visit. This team continues Cleveland Clinic treatment at home and helps patients learn or re-learn any skills necessary to stay safe and independent at home. As an LPN in the South Team, you will support the Akron/Canton region as well as supporting Medina, Wooster, Fairlawn and Hartville. In this role you will visit and provide care to approximately six patients each day in the comfort of their own homes. LPNs in this position typically start and end their day working remotely.
