Value Based Care LPN for DMC at Home Program

DMC Primary Care seeks an experienced Licensed Practical Nurse (LPN) to join our DMC At-Home Program as a Value Based Care/At-Home Program Nurse (LPN). The DMC At-Home team provides wrap-around care to a population of DMC patients who are not able to visit our offices in person.

As a Value Based Care Nurse, you will serve as a key member of the At-Home Program care team, supporting the Advanced Practice Providers (APPs) who deliver in-home medical services through telephonic care coordination and patient management.

You will play an integral role in transitions of care by contacting patients following hospital discharge, reviewing discharge instructions, performing medication reconciliation, updating electronic medical records, and facilitating communication between patients, providers, caregivers, and care management teams.
