As an LPN - Advanced Care Planning Specialist (internally called Facilitator), you will guide patients through meaningful, end-of-life care conversations and secure their medical wishes from the comfort of your home office.

This full-time position features structured 8-hour shifts scheduled Monday through Friday between 8:30 a.m. and 10:00 p.m. ET, with specific shift times rotating approximately every 6 months to ensure team balance.
