LVN Allied Health Professional

The LVN is a member of the Patient Aligned Care Team (PACT) consisting of (Veteran, Provider, RN Care Manager, Clinical Associate and Clerical Associate) and Expanded Team (Family/Caregiver, Social Worker, Pharmacist, Dietician, Internal and Community-Based Services) involved in providing care to the patient and in developing the PACT plan.

Major duties include:

• Demonstrates knowledge and ability to vary the order and sequence of assigned procedures based on patient's condition
• Uses judgment in selecting the most appropriate application of procedures and guidelines
• Reports and records significant patient information.
• Observes, identifies and responds to the patient's needs for nursing, medication, and equipment-assisted care and to recognize deviations from normal. Considers physical, emotional and social factors in modifying patient care plans.
