STATEMENT OF THE JOB
The Licensed Practical Nurse works as an integral member of the team providing services at agency locations across a range of service lines. The Licensed Practical Nurse is responsible for providing wellness services and health monitoring to meet the behavioral health and care issues affecting those served by the agency's Integrated Health and Wellness programs. The primary goal of health monitoring is to identify clients at risk due to poor physical health and medical conditions, improve their health outcomes through early identification and care, and prevent lowering their quality of life or early death.
