Job Description
**_Part time - 24 hours weekly_**
**_Days - 7am-7pm with every 6th weekend required_**
**General Summary**
Works collaboratively with the Capacity Coordinator (RN) to coordinate, communicate and predict impacts to bed management, transfers and admissions from all possible points of entry and provide general oversight of patient placement and patient flow throughout the Hospital/Health System
**Duties and Responsibilities**
**Essential Functions:**
+ Under the guidance of the Capacity Coordinator (RN):
+ Assists with patient flow coordination and all associated activities with physicians, clinicians and healthcare professionals, to ensure timely patient placement based on the patient's care needs, diagnosis and acuity.
+ Promotes cooperation and effectively communicates to appropriate professionals throughout the WellSpan Health System to achieve the objectives of the WellTrack Patient Placement and Transfer Center regarding th...
