Licensed Practical/License Vocational Nurse

The primary purpose of the PACT Licensed Practical/License Vocational Nurse is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the veteran. The LPN is the technical expert for performing, reporting, and recording assigned nursing care, including, but not limited to:

• Direct patient care.
• Rooms patients and completes all clinic reminders, obtains vital signs and completes any treatments as ordered by primary care provider.
• Phlebotomy outpatient blood draws
• LPN Clinics: Toenail Clinic, Immunization Clinic
• Quality Metrics- EQM/Population Health Metrics: Tracks panels metrics and implements strategies to manage fallouts via patient outreach or team collaboration.
