**Description:**

**POSITION PURPOSE:** Our Outpatient Intake Therapist will complete assessments for those seeking service.

**WHAT YOU WILL DO:**

+ Complete Intake Assessments
+ Complete biopsychosocial assessments to determine client’s strengths, needs, and diagnosis
+ Complete documentation within the electronic health record in congruence with organizational policies and regulatory requirements to ensure quality indicators and productivity requirements are met
+ Engage with the clients’ natural supports to strategize methods of supporting client goal progress and wellbeing and collaborate with members of the treatment team, community members, and third-party providers
+ Attend and participate in clinical, administrative, and community partner meetings
