Board-Certified Primary Care physicians throughout Montefiore's network are available in a variety of settings in the Bronx and Westchester County, including hospitals, clinics and individual practice locations.

Our ambulatory care centers, many of which have been designated as patient-centered medical homes, are highly accessible, provide proactive and seamless care, and engage patients in their own health.
