Integrated Care Clinician (Nurse) - Cwmbran - INTERNAL

The post holder will undertake telephone triage assessments of patients in an efficient way, advise the most appropriate intervention to meet the needs of the patient using best clinical practice, and adhere to Trust policy, protocols and guidelines. The role is carried out autonomously, assessing and advising callers within agreed guidelines.
