**Job Summary and Qualifications**

**Summary:**

**The Intake Coordinator is responsible for processing referrals for designated service line programs and admission scheduling directly or by facilitation to assure coordination of delivery of patient services. The coordinator facilitates contact amongst the medical, clinical and administrative staff as needed.**
