Work Shift
Sentara Leigh Hospital, located in Norfolk, Virginia is hiring a full time RN Case Manager for our Emergency Department.
Full Time 40 hours per week (5) 8 Hour shifts Monday - Friday
Case Manager - Assigned to the Emergency Department
A minimum of 12 months of nursing experience is required
Providing care to the patient and family over the entire inpatient stay
Acting as a clinical leader of the healthcare team
Working in a team of nurses and social workers, using each other's expertise
Helping to drive hospital and organizational strategic initiatives in always improving patient care
Responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS.
