The Palliative Care Manager is responsible for facilitating goals of care discussions, patient, family and interdisciplinary meetings and services for patients at the end-of-life. This role ensures effective utilization and monitoring of healthcare resources while managing interdisciplinary collaboration to achieve optimal clinical and resource outcomes. The Palliative Care Manager works closely with the Palliative Care team and the Manager of Palliative Care to assess, plan, implement, coordinate, monitor, and evaluate services and outcomes to maximize patient outcomes and ensure safe, high-quality care.
