This position is ideal for an LPN who enjoys working independently while advocating for patients and helping coordinate quality care within the comfort of their homes.

As a Home Health Licensed Practical Nurse – LPN Case Manager, you will provide home-based nursing care for patients recovering from illness, injury, surgery, or chronic conditions while working closely with RNs, physicians, and interdisciplinary care team members.
