*Health Quest Home Care - Home Health Aide:*

*Responsibilities:*
* Assists the patient with personal care activities including bathing, oral hygiene, feeding and dressing..
* Assists the client with exercises, ambulation, and transfer activities. This includes knowledge in use of assistive devices such as canes, walkers, crutches, and lifts.
* Independently assists the patient with meal preparation/cooking and/or feeding.
* Must have working knowledge of special diets, diet restrictions, and techniques to maintain sanitary kitchen conditions to prevent food-borne illnesses.
* Assists the patient with medication prompts to remind the patient to self-administer his/her medications.
* Identifies changes in patient condition that require notification to the patient’s registered case manager.
* Takes vital signs to include temperature, pulse, heart rate, respiratory rate.
