Position Summary:


We're looking for a committed, passionate and professional Healthcare Case Management Supervisor for our Jackson Heights location. The ideal candidate will be responsible for providing supervision to the healthcare case management staff to ensure quality care is provided to Apicha CHC's patients. Works closely with patients and the multidisciplinary care team at regular intervals for conducting, monitoring and evaluating of progress for patients to achieve positive health outcomes.





Job Responsibilities:

+ People management and supervisory - responsible for day-to-day patient care and quality services delivery to the patients under the healthcare case management.
