St. Peters Health Partners is seeking a full time Nurse Practitioner to join our Home Based Primary Care Team, delivering high quality primary care directly to patients in the Eddy Senior Living Communities throughout the Capital Region.

Since 2013, our specialized team has been serving vulnerable individuals who are unableeither temporarily or long termto access care in traditional practice settings. This model provides exceptional care while achieving high patient and caregiver satisfaction, improving health outcomes, reducing avoidable ER visits and hospitalizations, and delaying or preventing nursing home placement.
