Extendicare is seeking a full-time Licensed Practical Nurse (LPN) to provide goal-oriented nursing care and support. You will deliver patient care while ensuring compliance with care standards.
In this position, you will report to the Director of Care or Director of Health & Wellness and serve as a critical member of the inter-professional team. Responsibilities include patient assessments, care planning, and medication administration. Your role is integral to enhancing the quality of life for residents, ensuring their needs are met effectively.

• Perform clinical assessments and document care accurately
• Implement and evaluate resident care/service plans
• Facilitate care conferences and coordinate transfers
