Family Support Services (FSS) includes multiple programs serving pregnant and parenting adolescents and families in Southern Alameda County (Hayward, Union City, Newark and Fremont). Programs include: Targeted Case Management (TCM) Cal-Learn Alameda County Public Health Home Visiting Program (ACPH) Targeted Case Management (TCM) TCM provides Medi-Cal-eligible individuals with case management services to improve access to medical, social, educational, and other supportive services. Core services include needs assessment, setting needs objectives, individual services planning, service scheduling, crisis assistance planning, and periodic evaluation of service effectiveness and ongoing client needs. Cal Learn supports pregnant and parenting teens receiving CalWORKs/TANF in completing high school or equivalent education, achieving self-sufficiency, and strengthening family stability.
