Physician - Primary Care / Value Based Care
Interested in joining an Independent, Multi-Specialty Practice that is known for their patient centered approach, focused on value based care? Are you an experienced Primary Care Physician that has helped treat a variety of complex chronic conditions? Do you want to care for a diverse patient population?
A state of the art Clinic located in Brooklyn, NY, is seeking a Primary Care Physician to join their growing team. This Practice employs a wide range of Specialists (Cardiology, GI, Endo, Podiatry, OB/GYN, Rheumatology, Nephrology, Neurology, etc.).
Experience with value based care (risk adjustment, HEDIS/star measures, etc.) is highly preferred, but training is available.

Schedule:
- Monday - Friday, 8-4:30 PM or 8:30-5 PM
- No weekends required
