**Description:**

**Position Purpose**

In the Emergency Department (ED) at **Saint Francis Hospital and Medical Center** , the Nurse Senior – Navigator, informally known as the **Complex Care Coordinator,** plays a crucial role in supporting emergent and high-risk patients with complex medical, social, and/or behavioral health needs. The Registered Nurse (RN) in this role collaborates closely with the ED care team to deliver coordinated, patient-centered care and support safe, effective transitions across the continuum.
