Are you an RN or Social Worker interested in care management, case management, or care coordination? UPMC Chautauqua is looking for a Discharge Planning Associate to support their Clinical Care Coordination and Discharge Planning department! This full-time position offers a Monday - Friday schedule with weekends required on a shared schedule. You'll enjoy a set schedule.
