Job Description
* Southland Hospital, Invercargill
* Fixed-term, part-time role until September 2026 (0.4 FTE, 32 hours per fortnight)
* Salary band: $80,000 - $112,000 per annum pro-rata plus KiwiSaver

About the role
We are seeking a healthcare professional (either allied or nursing) to assist with the temporary increase in beds for the winter period.
The purpose of the Discharge Planner is to:
* lead the collaboration of the inter-professional team
* ensure all patients have a clear discharge plan to facilitate their safe discharge from inpatient services
* ensure all complex patients are reviewed within 12-24 hours of admission to facilitate care coordination and discharge planning needs
* reduce ED admissions through identification of appropriate community and outpatient pathways
