Health Advocates Network is currently seeking a **Dietitians & Nutritionist** to work at a facility in **Syracuse, NY.** These are _registry_ positions with our company. **$100 Sign-On Bonus**

**Pay Rate:** $50 / hour

**Shift details:**

+ Monday - Friday 8am to 4:30pm

Job Duties: Perform comprehensive nutrition screenings and assessments. Develop and implement individualized Medical
Nutrition Therapy(MNT) plans. Document nutrition assessments, interventions, and outcomes within the electronic medical
record. Provide nutrition groups, education and counseling to patients, families, and caregivers. Participate in facility-wide and
departmental Quality Assurance and Performance Improvement initiatives. Participate in multidisciplinary patient care rounds
and departmental meetings. Support staff development initiatives and contribute to improved clinical outcomes. Complete
Swallowing Screen as needed.
