An exciting opportunity has arisen for an enthusiastic Band 3 Community Support Worker to join our Community Response Team. If you are a carer, support worker, or someone interested in working for the NHS, we would love to hear from you!

Our Community Response Team covers South Warwickshire with bases at Stratford upon Avon and Leamington Spa. We work across the county supporting patients with identified health and social needs as a result of changes in their health condition. The service is committed to providing high quality care to prevent avoidable hospital admissions, enabling patients to remain independent in their own homes and facilitating safe discharge from hospital. The team works closely with social services, GPs, community nursing teams, patients, relatives and carers.
