Description and Responsibilities


Open Sky’s Community Supports Program is a Mass Health funded navigator service that provides case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting.
The Community Supports Recovery Coach uses lived experience with addiction and/or co-occurring mental health disorders to provide peer-level support and understanding to individuals served as they pursue their own personal path to recovery. They develop a relationship that is non-clinical in nature and is focused on overcoming barriers to recovery while linking individuals to recovery pathways and supports.
