Location: Singapore

Community Care Officers support elderly residents under the Enhanced Home Personal Care (HPC+) framework by conducting outreach, identifying needs, and coordinating care and support services.

• Conduct home visits and basic needs assessments
• Engage residents professionally to understand care and wellbeing needs
• Identify requirements relating to personal care, home support, and daily living
• Coordinate services with internal teams and care providers
• Maintain accurate records and reporting in line with programme standards
• Act as a reliable point of contact within the community

• Comfortable with community outreach and home visits
• Compassionate, observant, and responsible
• Strong interpersonal and communication skills
• Able to follow structured care and reporting processes
