About the Role
We are seeking an experienced and motivated Clinical Nurse Specialist or Nurse Practitioner to join our innovative SPARC service, delivering advanced, community-based care to older people living in aged residential care. Working autonomously within a multidisciplinary team alongside a geriatrician and pharmacist, you will provide early assessment, diagnosis and treatment of acute deterioration, support goals-of-care discussions, and coordinate care to reduce avoidable Emergency Department presentations and hospital admissions.
