- Conduct home visits and nursing assessment
- Perform nursing procedures and assessment.
- Triage clinical conditions and respond in a timely manner.
- Develop and implement patient care plans as part of a team.
- Manage and monitor a patient's needs.
- ADLs
- Maintain proper documentation.
- Assisting the Doctor as needed.
- Provide Caregiver Training
- Conduct peritoneal dialysis training in hospitals and at home.
- Call centre manning.
- Candidates are required to travel to the relevant locations as needed.
- Diploma or degree in Nursing.
- Singaporean/PR
- SN onwards
- At least 1 - 2 years working experience in acute hospitals.
- Registered with Singapore Nursing Board.
- Prior experience with dialysis machine operation is a bonus, but not necessary.
