Job Description
The Clinical Care Partner plays a key role in delivering high‑quality, clinically sound, person‑centred services to older people living at home. You’ll provide comprehensive clinical assessments, restorative care management, and expert guidance to Care Partners and consumers across the Support at Home (SAH) Restorative and End‑of‑Life Pathways.
This role suits a confident clinician who enjoys autonomy, problem‑solving, and supporting others to thrive. You’ll be part of a compassionate, values‑driven organisation committed to empowering people to live independently and with dignity.
What You Will Do
Clinical assessments and reviews for SAH and CHSP consumers, including urgent clinical interventions when required.
Restorative and clinical case management across the Restorative and End‑of‑Life Pathways.
