Pay Range: $26.40-$34.31*

We are seeking a dedicated Mental Health Treatment Coordinator to plan and facilitate comprehensive care for our patients. This role is essential in bridging the gap between inpatient treatment and outpatient recovery, ensuring every patient receives a holistic, multidisciplinary approach to mental wellness.

Key Responsibilities
+ Treatment Planning: Develop, implement, and update individualized care plans in collaboration with a multidisciplinary clinical team.
+ Therapeutic Intervention: Apply psychotherapeutic techniques in group settings and provide psychoeducation to patients and their families.
+ Care Coordination: Act as a liaison with outpatient providers to ensure continuity of care during hospitalization and seamless transitions upon discharge.
