The primary goal of this position is to assist patients in obtaining necessary services by providing information, coordinating referrals, and following through with specific treatment plan goals.
WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY:
+ Provides supportive case management necessary for individuals to access medical, social, vocational, educational, substance abuse and/or mental health treatment supports and community-based recovery services. Assists clients with researching and accessing needed services.
